St. Hope Foundation’s Dentistry Program provides a broad spectrum of services from routine dental care to complex restorations. We offer dentistry for patients with complex medical problems, including dental health screening for patients prior to transplant, chemotherapy or radiation therapy, management of patients at risk for sub-acute bacterial endocarditis, and management of patients with bleeding disorders. Our dentists specialize in endodontics and general dentistry.
Regardless if our dental patient is self-pay or insured, we strongly believe in excellent dental care treatment. Self-pay or uninsured patients rate are calculated on a very affordable sliding fee scale based on the patient’s income level. Proof of income must be presented at the time of service.
SHF Dental program accepts nearly all major insurance plans. To determine if we accept your plan, simply call us. On most cases, if we are not enrolled in the plan, our benefits team will immediately enroll into the plan allowing you to receive the dental care under your current plan.
We are more than happy to discuss the particulars of your plan with you before any treatments that you may undergo. For dental emergencies, we offer same day appointments. We value you as a dental patient, where you are more than just a number.

Your 1st Dental Visit
New patients receive a comprehensive consultation from the dentists explaining your diagnosis and treatment options. SHF utilizes electronic dental health records and the latest digital imaging equipment, so your dental treatment plan is securely and confidentially stored in a HIPAA compliant dental system. Because of this, the dentist can respond rapid to your dental needs and can offer treatment same day based on consultation. However, a complex medical history or treatment plan will require additional dental appointments to ensure the patient receives optimal treatment.
To streamline your dental intake registration, if you are a first time patient of SHF Health Centers, please complete the new patient registration link and select “dental” only. Complete the dental history of the intake to help us expedite your intake.
Please have ready the following items:
- List of Medications you are currently prescribed
- Copy of your dental insurance card
- Copy of your photo ID
- Proof of Residence (photo ID will have current address to suffice)
- Proof of Income (copy of w-2, paystub, etc.) ***Important for self-pay patients due to sliding fee scale determination is based on income level.
However, if you have access the medical program or another service of SHF, you do not have to complete the new registration. Simply call us and let us know you are an established patient.
